Description

A very simple WordPress contact form with Captcha validation. To add contact form at any page just add the following shortcode

[dialog_contact_form]

By default, It will just send email to E-mail address of blog administrator. To change email address go to Settings >> Dialog Contact Form and change email address as your need.

To activate dialog contact form check Show Dialog, a "Leave a message" button will appear at your site bottom right corner. Just click on it, a dialog contact form will appear.

Current input fields are "Your Name", "Your Email", "Website", "Phone", "Subject", "Your Message", "Captcha Code"

jQuery Validation Plugin has been used for clientside form validation.

Bootstrap Modals (modal.js) has been used for styling Dialog Contact Form
